Investigators seeking ethical approval should submit PDF formats of the documents listed below at the initial submission via e-mail (This email address is being protected from spambots. You need JavaScript enabled to view it. after paying the ERC fee (click the link; https://medicine.kln.ac.lk/prog/erc/index.php/erc-fees). Your submission will be included in the agenda for the next available ERC meeting, provided it is received by the relevant deadline and is complete.

Hard copies specified below should be submitted to the ERC office (Room no. A4 006) once the proposal is accepted.

1. ERC checklist (1 hard Copy)
2. One hard copy of ERC Application form
   2.1. One hard copy of ERC Application form (For Undergraduate Students)
3. One (1) hard copy of the complete protocol. The complete proposal should include a Participant Information Sheet (PIS), an Informed Consent Form (ICF), and data collection tools with date and version number in relevant languages.
4. One (1) hard copy of the single paged Summary form
5. Payment receipt if applicable
6. Signed and dated hard copy of a short curriculum vitae (CV) of the principal investigator- 1 hard copy
7. Short CVs of all investigators indicating their previous experience in conducting research- 1 hard copy
